HIV / AIDS Case Management
HIV / AIDS Case Management provides services to those clients who are diagnosed with HIV/AIDS and have MediCal.
Each enrolled patient is assigned a nurse case manager and social worker. Our program provides assistance for food, shelter, visiting nurse, care provider, and respite. Clients receive a health assessment by a nurse every sixty days along with a psycho-social evaluation.
Both nurse case managers and social workers help connect clients to community services such as Medi-Cal, SSI, SSD, MD referrals, Hospice, Easter Seals, in-home support services, immunizations and other services.

- Program development
- Bio-psycho-social assessments
- Suicide/crisis intervention
- Counseling
- Facilitating
- Care planning
- Oversight of multidisciplinary team participation
