Making a referral is easy.  Ask your health care provider to contact our central intake department at:  1-866-781-6774 or fax us the physician-completed form to : 1-877-756-3077.

Medicare usually covers professional intermmitent visits for patients who meet the following criteria:

• You must be home-bound
• You must be under a physicians care
• You must have a condition that requires the skills of a medical professional in order to improve.
