Provides assessment and treatment of patients requiring speech therapy services (e.g., improved swallowing, communication techniques) services within the homecare setting. Provides professional speech therapy visits in the homecare setting and directs home health services for caseload of patients and their families in accordance with physician’s orders through direct care, supervision and multidisciplinary team coordination.  Is a resource to the Field Staff in the areas of community rehabilitation and individualized rehabilitation needs of the patient.  Participates in the utilization review, performance improvement and interdisciplinary conference of the organization. This position reports to the Clinical Manager or Registered Nurses designee.

Current registration as Speech Pathologist.  Graduated from accredited school of Speech Pathology or Audiology.  Good organization and communication skill and ability to function with minimal supervision and provide a reliable means of transportation are required.
