Provides assessment and treatment of patients requiring physical therapy services within the homecare setting.  Provides professional physical therapy visits in the homecare setting and directs home health services for caseload of patients and their families in accordance with physician’s orders through direct care, supervision and multidisciplinary team coordination. Is a resource to the Field Staff in the areas of community rehabilitation and individualized rehabilitation needs of the patient.  Participates in the utilization review, performance improvement and interdisciplinary conference of the organization.

Current licensure/certification in California and CPR.  Graduated from accredited school.  1-2 years Phsyical Therapy experince, preferably in home care.  Good organization and communication skills and ability to function with minimal supervision.  A reliable means of transportation is required.
