Provides assessment and treatment of patients requiring social work services within the Home Health Care setting.  Provides professional social work services during visits in the homecare setting in accordance with physician’s orders and coordinates services with the multidisciplinary team. Is a resource to the Field Staff in the areas of community resources and medical social work needs of patients.  Participates in the utilization review, quality improvement and interdisciplinary conferences of the organization.

Current licensure/certification in California. Good organization and communication skill and ability to function with minimal supervision and provide a reliable means of transportation are required.
